Formal Referrals
(Performance-Based)
What is it?
This type of referral is generally considered a performance based referral and is often supported by workplace policy and procedure. This type of referral is indicated if an employer/referent is concerned or determines that an employee/member is experiencing life factors that are impacting work/training performance.
Why complete one?
Factors include, but are not limited to, chemical or mental health concerns. A formal referral is warranted when an employer/referent has reason to believe that if an employee/member remains in the workplace without intervention, there could be detriment to themselves, colleagues, or the organization.
What is the result?
This type of referral leads to TEAM reaching out to the employee/member and facilitating a thorough evaluation, which guides the following:
1. What interventions are needed to address factors interfering with work performance?
2. Does there appear to be a clinical reason why an employee / member could not return to their work place while participating in interventions?
3. If time off work is needed, what approximate time away from work force is deemed appropriate?
After that, TEAM keeps in touch.
After the evaluation is completed and recommendations are provided to the employee/member, TEAM will maintain involvement with the employee/member and provide compliance updates to the employer/referent regarding recommendation completion. This assures the employer/referent that the employee/member is accessing professional resources and yet still provides the employee/member with the necessary privacy to protect their health information.

This form must be received in our office prior to scheduling the member/employee’s appointment. For confirmation of contact or attendance at TEAM Corporation, an Authorization for Release of Protected Health Information must be completed and signed by member/employee and submitted. If possible, please complete referral form with the member/employee present and give a copy to the member/employee, then fax/email a copy to TEAM Corporation at (651) 642-1809 or [email protected]
